- Appointments are available by phone, text, or online request.
- Please bring your photo ID and insurance card to your visit.
- Copays or balances may be due at the time of service, depending on your plan.
- If you’re unsure whether you’ve been seen at MDI before, just give us a call—we’re happy to help.
