As a new patient at our dermatology clinic, we're excited to welcome you to MDI.
To ensure a smooth and efficient visit, we kindly request that you please bring any relevant medical records, including previous dermatology notes, a list of current medications, and your insurance cards.
During your first visit, you'll meet with one of our experienced dermatologists who will review your medical history, discuss any concerns or symptoms you may have, and perform a comprehensive skin examination. Based on this assessment, we'll work together to develop a personalized treatment plan tailored to your needs.

Before Your Visit…
At Midwest Dermatology Institute (MDI), our expert team takes a supportive, diligent, and meticulous approach to your skin care needs.
When you schedule your first appointment, you will receive a text message from our practice with relevant and important information.
We now offer a convenient and secure text messaging called Klara. The bubble at the bottom right of this page is one of the ways you can message with us. This is 100% secure and is only shared with our office. You will be asked to enter your name and date of birth for verification, no passwords or usernames to remember.
Upon check-in, please have your driver's license and insurance cards ready. Keep in mind that if the patient is a minor and not accompanied by their legal guardian, we will require a disclaimer in order to accept treatment. Patients under the age of 18 will require a guardian for the New Patient visit. We pride ourselves on delivering the highest quality of care and personalized attention to each and every patient.
