MOHS Micrographic Surgery at Midwest Dermatology Institute (MDI)

Precise Care for Optimal Results

Skin cancer affects millions of Americans each year, but thanks to advanced treatment options like MOHS micrographic surgery, there's hope for complete and precise removal with minimal scarring. At Midwest Dermatology Institute (MDI), Dr. Jeff Messenger, a board-certified MOHS surgeon, offers expertise and compassion in guiding you through this effective treatment.

What is MOHS Micrographic Surgery?

MOHS surgery, named after Dr. Frederick Mohs who developed the technique in the 1930s, is a highly precise surgical procedure primarily used to treat skin cancer, especially those with high-risk features or located in cosmetically sensitive areas like the face. Unlike traditional excisional surgery where a predetermined margin of healthy tissue around the tumor is removed, MOHS surgery involves the sequential removal of thin layers of cancerous tissue. Each layer is immediately examined under a microscope by the surgeon, allowing for precise identification and mapping of any remaining cancer cells. This process is repeated until no cancer cells are detected, minimizing the removal of healthy tissue and maximizing the preservation of surrounding healthy skin. MOHS surgery boasts high cure rates while minimizing scarring, making it an effective option for treating various types of skin cancer, including basal cell carcinoma, squamous cell carcinoma, and some types of melanoma.

Benefits of MOHS Micrographic Surgery:

  • Highest cure rate
    • MOHS surgery boasts exceptionally high cure rates, often exceeding 95% for most types of skin cancer. The meticulous examination of tissue layers ensures that all cancer cells are removed, minimizing the risk of recurrence.
  • Preserves healthy tissue:
    • Unlike traditional excisional surgery, which may involve removing a larger area of healthy tissue around the tumor, MOHS surgery conserves as much healthy tissue as possible. This preservation is particularly crucial for cosmetically sensitive areas like the face, where scarring can be a concern.
    •  By selectively removing only the cancerous tissue, MOHS surgery often results in smaller surgical defects and minimized scarring compared to other surgical techniques. This can lead to better cosmetic outcomes and improved patient satisfaction.
  • Outpatient procedure
    • In many cases, MOHS surgery can be performed as an outpatient procedure under local anesthesia, allowing patients to return home the same day. This convenience reduces the need for multiple visits and lengthy hospital stays.
  • Precise Tumor Removal:
    • Dr. Jeff tailors the procedure to your specific needs and cancer type.  MOHS surgery allows for precise identification and mapping of cancerous cells, ensuring complete removal while sparing healthy tissue. This precision is especially beneficial for tumors with ill-defined borders or those located in challenging anatomical areas.

What to Expect During MOHS Surgery:

  • Preparation:
    • Before the procedure begins, the surgical site is cleaned and prepared. Local anesthesia is administered to numb the area, ensuring that the patient remains comfortable throughout the surgery.
  • Tumor Removal:
    • Dr. Jeff starts by removing the visible portion of the tumor along with a thin margin of surrounding healthy tissue. This tissue is carefully labeled and mapped for reference.
  • Tissue examination
    • The excised tissue is immediately processed and examined under a microscope in an on-site laboratory. The surgeon meticulously examines the tissue margins to identify any remaining cancer cells. This step may take some time, during which the patient may wait comfortably in the lobby.
  • Additional Tissue Removal (if necessary):
    • If cancer cells are found at the edges of the removed tissue, the surgeon will precisely identify the location and extent of the remaining cancerous cells. Only the areas where cancer is still present are targeted for further tissue removal.
  • Closure:

    • Once all cancerous cells have been removed, the surgeon discusses the options for wound closure with the patient. Depending on the size and location of the surgical defect, closure may involve simple sutures, skin grafts, or other techniques to optimize cosmetic outcomes.
  • Reconstruction (optional):
    • In some cases, particularly for larger defects or those in cosmetically sensitive areas, reconstructive surgery may be necessary to restore the appearance and function of the treated area. This may be performed immediately following MOHS surgery or as a separate procedure.
  • Postoperative Care:
    • After the procedure, patients receive detailed instructions for wound care and recovery. Follow-up appointments may be scheduled to monitor healing progress and ensure optimal outcomes.
